Three pathways, all built for independence with support.

Each pathway is delivered around the same operating model: small team, named worker, structured plan, regular review. Whether the young person needs a bed tonight, a few hours of outreach a week, or an ISW report for a hearing — the same care goes into all of it.

Pathway 01

Community support for children & young people with SEND

Person-centred outreach for young people with SEND living at home with parents and carers — including those with Autism, Asperger's, Epilepsy, challenging behaviours or a mental-health condition. Sessions focus on building lifeskills and confidence, supporting access to education and the wider community, and giving parents and carers planned respite. We work alongside Local Authority commissioners, social workers and educational provisions on each young person's pathway plan.

What's included

  • Lifeskills and independence sessions
  • Short-breaks and planned respite
  • Outreach into community settings
  • Adjustments for Autism, Asperger's, Epilepsy and complex needs

Pathway 02

Supported semi-independent accommodation, ages 16–25

Our Ofsted-registered supported accommodation provides a structured pathway towards independence for young people aged 16 to 25 — including looked-after children, care leavers, and those leaving Youth Offending Services supervision. We accept planned placements and emergencies. Daily plans focus on education, training and employment alongside everyday living skills. Premises accommodate a mixture of female and male residents; matching is informed by risk, identity, sibling links and identified support needs.

What's included

  • Semi-supported living placements
  • Emergency placements at short notice
  • UASC placements (Unaccompanied Asylum-Seeking Children)
  • Mother-and-baby placements
  • Outreach and community support around the placement

Not sure which pathway fits?

Tell us about the young person — we'll suggest the right route.

Many young people we work with sit across more than one pathway: an emergency placement that becomes planned, supported accommodation alongside ISW assessment, or SEND outreach that wraps around a parent who needs respite. We'll talk it through honestly.
